So long story short, Toffee is getting on in years and has a lot of cysts which recently have been making her uncomfortable :( She is too old for surgery but is on Metacam 0.15ml once a day to help her feel more comfortable.
Trouble is it's effects seem to wear off through the day! Could I split the dose and give her half twice a day? Or in the short term give her a small dose of infant Calpol as well? x

You don't need to half the amount to give twice a day. Speak to the vets, if you speak to a good one they should give the correct dose for weight. I didn't manage to speak to a particularly knowledgeable vet last time I needed the same advice and they insisted that piggies can't have it twice a day. I didn't argue even though I have been prescribed it twice daily before. @furryfriends (TEAS) was really helpful with a guide to the dosage and I decided to split it myself though I did this knowing it would only be for a few days and not ongoing

How heavy is Toffee? 0.15 ml of metacam is a very low dose; you can safely give it twice daily and most likely go higher than that.
If the pain is getting very bad, discuss tramadol with your vet.

Thanks guys, you've all been super helpful :) I'll monitor her over the next few days and ring the vet if she is still uncomfy! @Wiebke Toffee weighs 1.13 kg
Ps I am currently in bed watching her on my phone downstairs with my WiFi remote camera, and I just saw her eat a poop :D

The dosage I’ve used for our boys is 0.4ml of dog metacam per 1kg, so this equates to 0.45ml for Toffee. I’m unsure if your dosage is lower due to longterm use? It seems to me 0.15 ml to be too low to be of any use. Tagging @pig in the city :)

When we saw the specialist last week she actually told me that for ongoing pain issues it is essential to dose twice a day as guinea pigs have such fast metabolisms.

I agree with all that has been said. An adequate dose is 0.4ml dog metacam per kg bodyweight twice a day.
Hope she is more comfortable now
