Mimmi's Weird Behaviour In Heat And Swelling

Mimmi has been in heat for a couple of days and she has been annoying her piggy friends unusually much with rumbling and everything else she could think of. This kind of "extreme" heat happens with her a couple of times per year and she goes so weird. She has always been very awkward and antisocial towards people during her heat (which is odd because usually she is a very friendly, happy and outgoing) but recently she has developed a very weird symptom.

So today was probably the last and the worst day of her heat and she was doing her best to make babies with her fellow girls. Suddenly she froze and turned very quiet and seemed very uncomfortable and possibly in pain. She was lifting and throwing her head and squirming and changing her position and lifting her feet all the time and making quiet grunting noises. When I picked her up I saw that her vulva was very swollen. The whole genital area was hard with swelling and quite weird looking (the photos are attached). I bathed her bum for a while and gave her pain meds. Now, two hours later she is totally herself again and out of heat, and her vulva is back to normal. This is the second time this has happened and both times were the same though the first time I didn't give her any meds.

Is this kind of heat symptoms normal? Or should I be worried?
Mimmi is over 4,5 years old.There shouldn't be ovarian cysts, she has been checked for them previously.

I would have that checked out with your vet in case there is an issue with her womb; or is she spayed?

She is not spayed. Yeah, I probably should get her checked. It's quite common after all for aging girls to have issues with the womb, isn't it? And Mimmi is my little "Monday copy".. I don't know is you can say that in English :D but she is someone you need to look after more than the others.
