Paperwork for adopting and surrendering

Can anyone who runs a rescue or fosters etc give me a quick list of things to include on adoption paperwork for my rescue.

Also do you get people to sign paperwork when they hand their pets over to you?

All rescues do and should use surrendering forms as well as the adoption ones

Included and the most important part would be the agreement that they will not breed/show/neglect the animal/s and also that in the case of the adoptive family one day having to rehome the animal the animal must be returned to the rescue or the rescue has to approve the new home.
 
Most adoption forms do also state the age and gender of the piggy and the fact that it was healthy at the point of adoption.

The adoption pack that I give to people who adopt my fosters from Sokel or Becklen include:
Personalised front page info sheet which says about Food & Veggies they like/dislike, other likes/dislikes. Extra information sheet. Settling in new Guinea Pigs sheet, Weight charts that I have kept whilst with me, Care Charts that I have kept whilst with me, Cold Weather Care Sheets, Hot Weather Care Sheets, Weekly check care sheet. Also the adoption form, that is signed and I keep.
I also have a big file with all the pigs details, DOB, where from, when I got them, where they go to etc.

It is essential that paperwork such as surrender forms and adoption forms are used when rescuing and rehoming. It is your only proof that the agreement exists and whilst 99% it will only be a formality there is the chance you may need to refer to the agreement in the future.
