Specialist Recurring Haematuria In Ovarian Cysts Sow

My 5 year old sow was diagnosed with ovarian cysts a few months ago, and had a bout of bloody urine (no obvious pain, still eating fine and no increased drinking) and was prescribed septrin (good for bladder issues thankfully since its one of few antib's she tolerates well). She had her first gonadotrophin injection 5 days ago and symptoms of her cysts have dramitically improved, appetite and poos have sorted themselves out too. Shes just been out for free range time for her dinner and has bloody urine again. Shes still bright and no signs of pain. I have some septrin so gave her the dose she was prescribed last time and half dose of loxicom to be safe. Does anyone know if this is going to be a recurring problem even if her cysts are improving? I know they come hand in hand but will there be anything else she could need done? Iv already cut out high calcium veg since her first episode of this and replaced them with different lettuces, and extra cucumber. Any help would be appreciated x
 
If it is the urine that is bloody and it is not bloody or infectious fluid comeing from the womb, has your vet considered a urinary tract infection or cystitis rather than an issue that is connected with the reproductory system?

Its definately blood in urine (discharge was yellow and was visible on the vulva) and urine was just all red (seen on puppy pad and on the underside of it too showing there was a normal volume of urine) so definately wasnt spotting from her uterus. It was diagnosed as a UTI and was assumed to be related to cysts.. The blood was becoming less prominent after 2 days of antib's and was completely gone after 4 days. Shes fine otherwise this time round too, but i sort of assumed this wouldnt be a recurring thing since she has responded remarkably well to her hormone injections. Would you suggest further investigation or wait until this clears up with anti bs and see if it stays settled once shes had her 2nd injection as she has only had half treatment so far? Obviously i'll be speaking to vet tomorrow anyway to confirm I'm providing correct treatment and if so, to get more of the septrin. As i said, shes very bright and happy still!
 
hi there , dificult to say, but i were my pig ,i would talk to my vet about cutting out
all treatment , as ypu know anti bs only have a limerted time in which they are usfull, and
then all you are doing is vompermising her imune systim .

if she were yonger , yes! but at 5 years , :hmm:

How long was her course of antibiotics? There are also several different issues with similar symptoms wrapped up under the heading of UTI, which is a bacterial urine infection which can be quite persistent, especially when the immune system is a bit under pressure.
Cystitis is an inflammation of the bladder walls that often doesn't react much to antibiotics. it this recurring, you are talking about interstitial cystitis, which is something you rather manage than heal.
I would definitely speak to your vet about it; it is easier for him to judge what is going on. Between the septrin and the loxicom (which is the anti-inflammatory), you have addressed either issue for the moment. I guess that you have had an x-ray re. cysts, so you know whether there is any sludge etc. in the bladder? A much rarer but somewhat trickier to treat option is if the problem is tracking back as far as the kidneys.

First treatment was for 7 days and worked. This is only the 2nd time and i have given first dose of antib's tonight. Also the hormone injection she had was the Gonadotrophin releasing hormone and not the HCG which is the one that supresses the immune system. Shes had 3 ultrasounds, not xrays, one of which was with a specialist but there was no problems with bladder and sludge wasnt mentioned.
 
lm not going of topic , but have you or your vet looked into magnesium to disperse the sluge ?

There was no evidence of sludge when the specialist scanned her 5 days ago so dont think thats causing the problem but thanks x

I'm not sure that UTIs and cysts go hand in hand, so I would look at it as a separate problem. It could be that a longer course of Septrin is needed, or a different antibiotic It sounds like you have covered the usual investigations. Regarding a low calcium diet, have you considered the drinking water and any nuggets? Worth a look, possibly. let us know how you get on.

She had a treat of chard and a small amount of lambs lettuce as a treat the day before it started as id been cuttin out high calcium.. I dont know if this has set it off. I'm not sure about nuggets etc.. Shes on blackcurrent and oregano burgess excell pellets. I also read before bottled water isnt a good idea as has so many minerals they done need. She is otherwise bright and eating well and the blood is now only a hint after 24 hours of antibs as apposed to bright red it was last night.

- rescanning the cystic ovaries in a month or so to see how they have responded, not all cysts will respond to the injections

Crunchie has several small follicular cysts almost resembling a bunch of grapes. 48 hrs after her first hormone injection her aggression/hormonal behaviour had completely gone, appetite went back to normal and poos became normal too. She was still passing small amounts of yellow vaginal discharge the same night the haematuria started. The next day i actually saw her pass it on a puppy pad on my lap so blood is definately from the urine. Her urine is now a light brown colour not profusive red blood urine. She is still bright and eating. She is due her second and final hormone injection a week today, i will take in a urine sample for culture and i will have her ovaries scanned a few weeks after that. I going to continue antib's for a 10 day to 2 week course instead of 1 week. I have cut down the calcium intake in her veggies and ordered vitacraft prebiotic pellets online as they are lower in calcium and higher in fibre. I am aware of not letting calcium intake getting too low. If she has another episode of haematuria after her treatments over and on new food then il request investigation. Thanks for your help x
