Recurring wound

Wondered if anyone can help.

I have two 4 year old male Guinea Pigs. One of which suddenly started being really wet underneath and very smelly. I took him to the vet who diagnosed "Boar Bum". After the faeces was cleared however there was no improvement. A second vet visit found a lump in his bladder. This responded really well to antibiotics and the problem seemed to disappear. The antibiotics stopped but the problem came back, we tried a longer course of antibiotics which again seemed to work. Then I noticed a very small sore spot near his scrotum. I took him back to the vet and a different type of antibiotics was tried. This made no consistent improvement in the wound. Some days were better than others. So an Xray was done and found a collection of stones in his bladder. The vet said that he thought that the stones were causing an infection and he must be traumatising himself. He therefore had an operation to remove the stones. The vet was surprised how few stones there were and was cautious in saying that he would now be ok. He talked of contacting an expert to try and shed more light on the problem if it continued. Two weeks later he seemed fine and the vet said that we could be quietly confident that he will now be ok. However the wound has on a couple of occasions since then come back, fairly slight and to be honest Ferb (the piggy) seems really well in himself, as he has done through the whole ordeal. Its strange as it is always in the same place and it disappears and reappears really quickly. Once it disappears it leaves no mark like there was never anything there. As he presents so well I am trying not to worry too much - he's been through so much already. I wipe him down with a Guinea Pig wipe to keep it clean an it then clears up and reappears a while later. I don't think it's a through fighting with the other one either as they have the odd spat - usually settled by the "who can get their nose higher" competition, and despite what he's been through he usually gets the upper hand! Don't know how much more the vet will be able to help and if any treatment will be more distressing to him in any event.

This may be unrelated but the most recent occasion was after I brushed him - I hadn't brushed him much lately as he hates it and I thought he has had so much poking about recently I wouldn't fuss. I had to cut out a few matted bits and he didn't appreciate the attention! so wondered if it may be stress related. Anyone else have any ideas?

Could you post a picture of his wound? It could be stress related, but there could also be a mites, fungal and/or microbial problem in the wound area that causes him to bite/scratch it open again.

I am sorry, you sound like you have been through the wars. In order to help prevent a fresh build up of bladder stones and cystitis, you may like to have a look at the low calcium diet in our food section; some people swear by the ph : ca ratio diet instead (also found at the top of the food section) although I have found the latter a lot more difficult to manage and have made better experience with a low calcium diet.

thanks for coming back to me. True to form it is almost clear today after looking quite bloody & sore yesterday. I'll keep an eye on it and will take a photo if (or when) it flares back up.

unfortunately piggy has had a huge flare up. A couple of occasions the wound reappeared but never enough that I could get a clear picture. Then yesterday I noticed marble sized lump in his lower tummy. Guinea is happy on himself and eating well. Today whilst clearing his poo as usual, I pressed the
lump and a yellow substance came out at the spot where the wound usually appears and the lump has gone down a little. Spoke to vet on phone and he thinks it's an abcess. Can't help thinking there is something underlying causing all these problems but can't see how it can have gone unnoticed on
the xray. Any help appreciated.
 
If there is yellow goo coming out of the wound,it does sound like an abscess. I have very limited experience with abscesses, but I do know that he will need antibiotics for the infection (Zithromax is very good for abscesses, but sometimes it is hard to get a vet to prescribed because I think Zithromax isn't technically for pigs. I personally have had good luck with Bactrim with George's abscess). You might also have to have the vet drain the wound and rinse it out (we used chlorhexhidrine on george) and keep the wound open so it can heal from the inside out. You are right - there has to be an underlying problem. Good luck, and I hope you and your vet can find a good healing treatment!
 
thanks for all your replies. thought I'd give a quick update as it may prove useful to others. Vet thinks it's an abscess caused by the breaking down of his sutures from his op. that he has had a reaction to them. we tried antibiotics but they didn't work so the injected "Gentimicine" into the lump which seems to have worked. still has a couple of small lumps but vet said that piggies develop a thick skin around abscesses which effectively isolates them so probably not to worry if he is ok with it. weirdly his pooping problem seems to have improved. hopefully this is the end of a really long and painful (for piggy) and expensive (for us) problem. hope this helps anyone else facing something similar.
 
Glad he's on the mend. Gentamicin is a very strong antibiotic and injecting it will get it right to the heart of the problem instead of all through his system and will avoid some of the not so helpful side effects. Guinea pigs can be resilient so hopefully he continues to do well.

An abscess would be unlikely to show up on an x-ray anyway as it is a soft tissue infection issue so please don't worry that it was missed because of this. Abscesses can hide well and pop up again later which sounds like what your little lad has had.
