Cheilitis is usually caused by feeding fruit and tomato (which are technically a fruit and not a veg) either too often or too much at once. The acidity in them can cause tiny cracks in the skin through which bugs can enter. Treatment depends very much on what your vet suspects - bacteria, fungal or - if you are unlucky - one of the real nasties.
Please do not feed fruit or tomato ever again to any guinea pig with cheilitis and ideally never give a very small quantity more than twive in a week as a special treat.
