Advise needed about boars poorly privates!

I am currently fostering a male guinea pig whom I have named Simon. Firstly I would like to mention Simon is currently under vet care but we have got to a stage that nothing seems to be working, so I thought I would mention on here to see if anybody has dealt with or seen a similar problem.
Simon was dumped in the rescue centre along with Sally, in a cardboard box with a note explaining they could not look after them anymore. Simon is neutered and looks a very mature age. He was brought in extremely underweight with a very severe infection in his privates (penis).
He was straight away put on a long coarse of baytril, and each day he was massaged and cleaned in the area to remove the horrendous amount of yuck that had built up in the area. This cleared the problem up, and his weight was increasing steadly, but the moment he came off antibiotics within a few days it was back and just as bad. He was started again on another long hit of antibiotic but this time septrin. This again cleared the problem until his medicine finished and its back. This is where we are at at the moment, in
himself he is eating well and lively but obviously this is going to be short lived unless a fix can be found, the next step with the vet is to take a swab and see if that can answer any questions but that will take time, so I have turned to here to see if anyone has any suggestion or dealt with anything similar.
 
Sorry Ive never dealt with such an ongoing problem. Best of luck with it all, i do hope Simon gets better soon. Lots of love to him. A swab would what i would suggest, but it sounds like your vet is on it.

Do pigs by chance need anything like pro-biotics? I am wondering if he has something similar to a yeast infection? I am by no means a vet, but just the impression that Simon gave me (I'm an animal communicator).
