It could possibly mean he scratched himself and the scab is healing. Or if there is another piggy it could have scratched it. If it's flaky skin it could mean there is just dry skin. I couldn't see the photo so I'm not sure. I would need a picture to be sure.
Hi! Please have it vet checked as it needs a hands-on examination. It could be a huge sneeze of something that gone into his nose or it could be potentially a fungal problem. We cannot decide just from a picture.
Does the cavy live with any other guinea pigs? Another guinea pig must have bit him or he scratched himself. Either way, it is healing and it should be fine after awhile.
