Dental disease

I have been trying to manage my guinea pigs dental disease for the last couple of years. His incisors broke and never grew back right. I have been trimming them as needed. The root of the top and bottom incisor on the left side are both affected. The teeth are rotten and the roots severely damaged. We tried antibiotics for about a month to see if that would improve anything. It did not. I went to a new vet for a second opinion. She recommends removing the affected incisors since his mouth is very painful. She seems very confident in the procedure. I am always nervous about anesthesia with my piggies. He is 5 years old. Does anyone have any experience with this type of thing? He is on metacam and gabapentin for pain. He is eating hay and veggies. I have read that jaw fracture is a risk as well as infection or abscess from the procedure. Any input is welcome. I feel like I want to do it, but worry about losing him or making things worse as he is stable now.
 
Does he have all four incisors at the moment? If these two incisors are infected and possibly a bit loose, it would definitely be worth getting them removed. One of our piggies had an incisor removed last week. His had become wobbly and had infection in the tooth root xx
 
He has only had 2 good ones for almost two years. The other 2 are just terrible. They are loose and rotten. They only have grown to maybe half the normal size. I am leaning towards removing them. I just worry about anesthesia.
