I’m taking my Guinea pig to the vet tomorrow any thoughts on what this could be?

Please have him checked for ringworm. It is the most contagious fungal skin infection and can be transmitted between humans and other species pets. It is also sadly very common in pet shop guinea pigs.

Ringworm: Hygiene And Pictures - hygiene is absolute key for beating ringworm and preventing it from spreading. In a dozen years on this forum we have worked out about pretty much every possible angle of transmission and how to address it. Getting hold of a good disinfectant (brand names in the guide) is the best investment you can make!
