Hi, my guinea pig is nearly 3 months old and it was purchased from the pet store. One week ago, he was breathing very hard and took him to vet and diagnosed as URI. Today I noticed he got a white bit on his skin, anyone knows what is that and how to treat?

Hi and welcome!

I am sorry that you are having such a rough start. Please have your piggy vet checked again for either fur/hay mites, fungal/ringworm or mange mites. They need different, but good quality treatment, so it is much better to treat the appropriate full course than treating on spec with cheap, low dosed shop products that don't do the trick - in the end your piggy suffers more and you end up paying more.

Any of these can hit when the immune system is lowered through illness or stress. Just from your picture, I cannot give you a firm diagnosis.
We have got tips for caring for a ringworm piggy if that is what is diagnosed to prevent the infection from spreading and recurring, as it is the most trans-species infective thing your pets can catch.
If your piggy is diagnosed with hay/fur mites (which fix their tiny egg cases to the hays at the bum end and that usually come in hay), please ask your vet to treat with stronghold. This is currently the most effective treatment in getting on top of the new strain that has been increasingly become widespread in the last couple of years.
If it is mange mites, then a full three round course with a high dosed ivermectin product is the best.
 
He is currently on antibiotics and metacam for a week already. I rang my vet and they said Monistat or Lortrimin cream can help if it is a fungal

But is it fungal? It won't help if it is hay mites, which in your piggies coat are not at all easy spot. It will also not do the trick if it is ringworm.To me, the spot rather looks what my newly adopted Nye came with - and he had fur/hay mites, which are tiny specs on the bum end; you can't see the mites themselves, just egg cases which are fixed to the hairs. The skin in the balding patches looked scurfy like rather like yours.
