Incorrect Dosage Of Fortekor?

I am up to my eye balls at the minute with 2 sick guineas. Stressed is an undersatement. Fudge, my 6.5 year old heart disease and possible renal failure girl, was attending a local vets. When i wasnt getting answers i travelled further afield to see a more specialist vet. He put fudge on fortekor and frusemide. The fortekor was made up into a solution, of two 2.5mg tabs in 5ml water, which is 1mg/ml.the dosage was 0.5ml once daily so she is on 0.5mg daily.

From research, i thought this was a low dose.

The vet works a tuesday at one practice and a wednesday at another. I contacted him last night to tell him Fudge had responded well, more active and popcorning as days went on, but by sunday her activity slowly started to decline. He advised i increase the dose to 0.75ml, which would be 0.75mg a day.

He then asked me what dose i had be given as he didnt have the file infront of him, and when i told him the solution was 5ml, he said it would have been one 2.5mg tablet in it. And if it was 2 tabs, it would be 10ml. So either hes given me a double dose , or the labels wrong, or he got confused last night. I have contacted the vet and ofcourse he isnt in today! I need more of the meds but cant call my own vets for a prescription as basically i dont know what shes to get!

It sounds like he intended for her to have 0.5ml of a 0.5mg/ml solution, which is only 0.25mg a day. Can anyone conform if this is too low? Instead shes had 0.5mg a day up until last night where she had 0.75mg.

Her breathings worse this morning but thats more due to the reducing of her frusemide- he advised me to give 2mg 12 hourly but it wasnt enough- i've been giving her 2mg 8 hourly which had it under control. I put it back to 12 hourly last night as advised, but it isnt enough.

Any help would be greatly appreciated, i really am at my wits end as i've felt like both my girls dont get taken seriously wherever they go.
 
@helen105281 as she is very experienced in heart piggies. Hope your girl is ok, generally piggies can cope with much larger doses of meds than are given, so try not to worry too much in the meantime x

The strange thing is last night, he said "i mean i've already given her a high dose so i dont want to increase too much".

If his intended dose of 0.25mg is high in his opinion, what have i done by giving her double that all week, and then triple last night?!

I'm so worried.
 
Please don't blame yourself, I am sure Helen will know the dosages when she comes along. It sounds like your girl is quite poorly, is it worth getting her in to see the vet today if you can & discussing the dosages with him? x

I'm going to the vets today with my other sow to arrange surgery. I need to request the medication from them for Fudge but cant do so until i know the dose. Dont want to take her down and stress makes her breathing worsen. Had to top up her frusemide a little, shes breathing much better. And had a wee "mad half hour" running riot and popcorning. Hopefully someone calls me back before 3pm so i can get to my usual vet.

If you have dosed higher don't panic, we once gave ours too much as Ross had worked it out wrong and they were ok, and it was for longer than one night. Just keep an eye on her.

I'm sorry it is very confusing.. But clearer now. Managed to speak the vet that prescribed the fortekor- he made the mistake on the phone last night- not when he made the medicine.

So he originally prescribed 0.5ml of a 1mg/ml solution one daily- which is 0.5mg a day.

Hes now advised me to up it to 0.75ml which is 0.75mg a day. He says do this for 5 days, if she improves i've to drop it back down to 0.5mg.

Surely if its helping, i should stick to it? Why is he saying 0.5mg is a high dose which, from my research, its very low?
 
I'm in my usual vets now waiting to be seen with crunchie- will ask about fudges fortekor and see what they think.

But I'm pretty certain they wont want to step on his toes, even if they dont agree. Hes a well known man in the vet world and i highly doubt anyone will question him!

Just home, They thought the fortekor for a piggy was "daring" so they have no idea about dosage! I got more medicine at the specialists recomended dose of 0.75mg a day then reduced to 0.5mg if improves. Crunchies scheduled for surgery tomorrow.. More stressed now!
 
could he mean a higher dose in her case? because he's the one who has actually seen her in person and knows her history and weight/condition etc? if he's well regarded and piggy savvy then maybe go with it for now. you can always update him regularly and adjust the dose from there depending on how you feel she is doing along the way? :)

i hope fudge continues to do well and crunchies surgery goes well tomorrow :)
 
It does still seem a low dose to me, but it really depends on the vet and whether they consider the fast metabolism of the pig. My own vets put mine on a range of doses from 1/8 of a tablet twice a day. The tablets are 5mg so this is a total dose of 1.25mg a day. It is given in 2 doses as it is likely to wear off half way through the day. I have access to a heart article that may help and will message you about it in a mo. It may give you something to discuss with your vet when appropriate and is from a piggy website called TKCC. It has been approved by a selection of vets and references the doses in different formularies that the vet will have access to.
