Middle Ear Infection Guinea Pig Not getting better.

Hello everyone seeking advice and support for my guinea pig.
About a week and a half my guinea pig was diagnosed with middle ear infection. They prescribed him with oral meloxicam and oral enrofloxacin.
After 2 days with the medication he stopped eating and pooping. So I tried critical care, but I couldn’t get enough in, barely can get 3 ml. The vet said it was the side effect of the medication he is taking, causing loss of appetite. He was prescribed with oral metoclopromide and baytril ear drops since they found discharge in his ear. They said to stop metoclopromide medication once he was eating and popping normally. But his appetite never went back to normal so I continued with the medication and critical feeding (would roughly get in 5ml). He would mostly eat a bit of hay and a bit of veggies. So he was now having 3 oral medications and the ear drops. Along with weekly probiotics. He finally started pooping.
After couple of days eventually he started peeing orange! So I called the vet, and they said to stop the oral enrofloxacin and meloxicam. They were more concerned that we wasn’t drinking enough water or eating and hope that removing these 2 medications will help his appetite return. So since last night he is only taking oral metoclopromide and baytril ear drops for the ear infection. I syringe feed him and give him water through syringe both forcefully. I still don’t see an improvement, other than I no longer see orange pee anymore. He still continues to poop at least. I would say his activity has decreased since I stopped the meloxicam.
He mostly hides and is not eating hay as much anymore. I wouldn’t say he is lethargic, it seems like he is mostly resting and avoiding me. But I am concerned that he is still not eating after stopping both medications, not sure if it takes longer for the side effects to wear off but it’s about to be over 24 hours. I am at a loss I’ve already spent over $1000 with all his treatment and don’t have enough money to continue taking him to the vet.
Does anyone have any stories of going through something similar or have any advice of what I can do?

Middle ear infections are difficult to treat due to it being a walled off area.
Your piggies issues with not eating are most likely due to the infection and pain and I am absolutely stunned you’ve been told to stop the painkiller in particular.
Antibiotics can upset the appetite but a good probiotic (as you’re a US member I think benebac is one most commonly used) and plenty of syringe feeding is needed to combat it.

Middle ear infections are very painful and impact everything to do with moving the jaw - hence his reluctance to eat. Stopping the painkiller meds can mean his pain intensifies which will make him even less likely to eat.

What dose of meloxicim was he on?
Was it twice a day?
Was it dog or cat version?

Giving ear drops can make things more painful.

You need to be getting a minimum of 40ml of syringe feed into him per day. Ideally you aim for 60-90ml per day though. The less he takes at each sitting, the more sittings are needed each day.
You need to weigh him every morning so you can monitor that he is getting enough syringe feed to replace his lost hay intake. You’ll know it’s enough as his weight will be stable each day. Any loss then he isn’t being fed enough.
You can also syringe some water to him with each syringe feed.

The pain meds would be out of his system after 12 hours. The antibiotics will also be wearing out now.

I would get him back to the vet, or a different vet, to get him back on a decent dose of painkillers. Painkillers need to be twice a day and they can tolerate pretty high doses comparative for their size.
There are other types of pain meds available is he is genuinely having an issue with meloxicom (as I say though it is mostly very well tolerated)

Hi thank you for your advice! Literally last night he drank water by himself for the first time in a week and eating more hay than usual! Today in the afternoon he started asking for veggies and treats, and today he ate critical care food by himself. So his appetite is definitely coming back. He is still not eating his pellets, but I am thinking of getting him back on the pain meds and hope that he’ll eat more by himself. But it seems stopping the oral antibiotics helped a lot!
 
Antibiotics can knock their gut.

Make sure you are weighing him because you cannot gauge hay intake by eye. The weight checks are essential to know he is actually getting enough and where you are in giving him the syringe feeds.

Not eating pellets at this point isn’t of concern - they don’t need pellets anyway.
