I honestly don't know........ maybe he is rubbing the hair off?
I'd be taking him for a vet check to make sure nothing is wrong with his skin.

Adult Guinea Pig
One of my girls had a cyst that burst under her chin, and as it healed up it left a bald patch that looked the same as in your picture. She is now fine and back to normal. If you are worried have her checked out by a vet.
