My male guinea pig has this? Does anyone know what it is and do I need to take him to the vets?

It is rather difficult to see what exactly is wrong from your picture, as it is not quite sharp, but it doesn't look quite right. Better have him checked by a vet who can really handle, diagnose and treat your boy and not just make guesses.
