Poorly Nutmeg? He's very small and recently grumpy

Hello, I'm a bit worried about my piggy Nutmeg. He's about 5 months old and he's 620g. I have another pig the same age and he was 700g the last time I weighed him. There is a clear difference in their size and when I hold Nutmeg he feels really thin and quite boney (especially around his hips.) I've been separating their food and making sure I hand feed Nutmeg etc to make sure that he's getting enough and not being bullied. He definitely eats but he seems to eat really slowly and seems less interested than Ginger. I thought that I'd just keep an eye on it but recently he's become really grumpy. He used to love being cuddled up and would fall asleep on my lap and wouldn't mind being picked up. Now he jumps away if I try to touch him. He also seems really grumpy with Ginger and keeps turning around as if he's going to bite him, although he never does. I was wondering if this could be a sign of mites or a fungal infection?

Last week he had this on his ear (sorry for the bad photo)

It looks a bit better now but is still slightly white. I took him to the vets about 2 weeks ago and the vet said he looked a bit smaller and thinner than he'd expect but didn't mention much else. I think next time I'll definitely take them to another vet as it wasn't very thorough.
 
I would get another opinion from another vet that is cavy savy. It looks like fungal on his ear, but get it checked out to be sure. The fact that he is grumpy too leads you to think something is a miss.
