Hi and welcome

Are you weighing daily first thing in the morning to monitor any potential weight loss since you cannot judge the hay intake with your eye. It makes over three quarters of what a piggy eats in a day. He can easily just sit in there without eating much.
This guide here tells you what to look out for when health monitoring so you can put any developments into the correct perspective. This is then going to help your vet as well: Weight Loss Explained: BMI, Weighing, Poos and Feeding Support

Mystery weight loss is the least favourite symptom because so many different issues can hide behind it without any other clear symptoms. The majority of these turn out either to be a problem in the mouth (teeth or oral thrush/sores) and the digestive tract or to be caused a pain issue somewhere. If the pain is internal or diffuse like from an internal growth or an internal organ going wrong etc., it can be extremely tricky to diagnose.

All you can do right now is to switch to daily weighing and stepping in with feeding top up support when needed and to keep an eye out for potential helpful symptoms to develop. The first link contains access to further helpful information and support care links if you need to step it up.

I am very sorry; I know only too well how frustrating it can be when there is nothing obvious. Unlike our larger pets, guinea pigs are prey animals so they are wired to suppress any symptoms for as much and as long as possible, which most decidedly doesn't help us owners or any vets! :(

It might be worth a second opinion from another vet.
What checks did your vet do on Leo?
