Some Advice - Massive Weight Loss With Infection/pain Issue

Hello everybody!
I need some advice about Midnight.

So, as some of you may know, a few months back he was diagnosed with chronic bladder problems. He was prescribed potassium citrate, as well as metacam and a consistent antibiotic for pain and his bloody urine (the antibiotic seemed to help that). He was doing very well, with minimal bad days. About a week ago, I noticed that he wasn't doing well. He was walking funny and just didn't seem himself. I checked under his belly to see if something was going on to make him walk weird. I noticed he had a lump in his groin area. I immediately made him an appointment. While at the vet, I realized he also had lice, something he has never had and I had never dealt with, though I know it's common. That meant I also had to bring in my other pig, Mocha. The vet noticed that Mocha didn't seem to have any lice, even though they're cage mates. This led us to realize that Midnight must have an extremely poor immune system, probably chronic, and that is why he gets sick all the time.
She thinks the lump is an infected testicle, and he is now on baytril as well as the other antibiotic.
Here is where I need the advice: since that day at the vet, he has declined rapidly. He has lost so much weight that I can feel every bone in his body, and if he didn't have fur, I'm afraid of what he would look like. However, he's eating and drinking normally. He sleeps all the time and seems to be in pain. He's always hunched up, never stretches out like he used to. Often times I feel like he is just not there. I just don't know what to do. I don't want him to suffer, but I have seen him make miraculous recoveries before. The antibiotics are making the swelling in his testicle go down, but the weight loss has me very worried. When did you know when your pig had reached their time? I'm struggling with the idea that I am in control of when he dies. How do I make that decision?

Please start topping your boy up with syringe feed asap! About 80% of the daily food intake should be hay, but you cannot control the intake other by weighing daily at the same time. It is often the first food group to be dropped when a piggy is in pain. Please get a painkiller from your vet!

There can be an underlying issue, but you will only know if it is just the pain or if something else is going on if you start syringe feeding. Hunching up means that the pain is located somewhere inside the body.

He is on Metacam twice a day, which makes for four medications twice a day.
 
I've just realized he probably has malocclusion on top of all of this. I feel like the worst owner. It's constantly something and I feel like I can never be on top of all the things that seem to go wrong.

It is likely one underlying problem that is causing all the others. :(

I have started syringe feeding Oxbow critical care. He's pretty much eating it on his own. I don't have to force it. I've already noticed his breathing is less shallow, so thank you so much for this advice. He is also eating veggies and pellets on his own, but I've been feeding him whatever he will eat every two hours.
