After observation and action it turns out this is exactly what is going on. This morning I checked him even though he showed no signs of a buildup, but sure enough, that pouch was full of unexpressed material, it just hadn't built up enough to protrude. I left it in the cage for him to get the nutrition; I don't know if he ate it or not. It might be my imagination but it looks smaller. I'm not going to hand feed it to him, at least not right now, but I will leave it instead of wrapping it and tossing it.
 
Great news! Glad that you could find out what is going on. Sadly there is no cure for impaction - or the smell!
