Flaky skin/ongoing drandruff

I have a boar who has had ongoing skin issues over the last 12months. I've been to the vet, a few times, they've done a physical exam but no skin testing.

He has flaky skin at the base of the hair follicle, especially around his rump area. When I groom to dislodge it, he tries to excessively groom my hand, so I know it is making him itchy. He also has white crusts around his eyes, and his right nostril. If you gently scratch the fur around his nose and eyes, more white flakes come up in the fur. He has no significant hair loss, other than what comes out when grooming.

I have treated for mites with both ivermectin and revolution kitten, he has been bathed in Fidos Free Itch, malaseb, and gorgeous guineas lice and easy periodically over the last 12months. I'm just not sure what else to do as some mornings, the crust in the nostril is a lot.

He also has fatty, or pea eye on that right eye, and I've thought perhaps that causes some of the crust issues with hay dust and dirt entering the eye and not being able to clear properly? It is also his right nostril that crusts.

It isn't ringworm, there are no round patches, however could this be some form of systemic fungal issue? You would think with multiple ivermectin topical treatments that mites wouldn't be an issue, lice neither after bathing (yes, spaced apart from the ivermectin treatment), and malaseb is used for fungal.

He has a black coat so the flakes look white against the skin, but when they comb out they are very dark, maybe black. He is quite obviously itchy when you groom his back end, or when you pat him. Again though, no major hair loss, but maybe a bit more breakage than usual.

Does anybody have any suggestions? I'm limited with exotic vets where I am, they are 2-3hrs drive away, and so far haven't really taken the issue seriously apart from recommending revolution, malaseb and lice baths. Could the eye and the nose be the same thing as the rump? If a fungal problem got hold enough, could it cause issues with crust around the eyes and nose? I just want to clear this up so he is happy and well!

I should also say that he has unlimited access to hay, i buy local bales of Rhodes for him to burrow in, and oaten goes in the hay bags. I've heard hay can bring lice, and also that the dust can cause respiratory issues, which may cause the eye and nose problem?

I have 3 other boars, this guy has neighbours but prefers his own mansion, and none of the other guys seem to have any problem. I'm so lost and the stress of trying to figure it out is wearing me down. Any and all advice much appreciated 🙏
