I haven’t been on in a while but just spent 5 hours at an emergency vet last night and need some advice. Exotic vets are hard to find in my area and I called 6 emergency centers before someone would see me.
5 year old female piggy, not spayed lives with another female same age. Been keeping an eye on her eyes for the past week as she has had some crust and not been as spunky as herself but eating well. Yesterday I got home from night shift and only had bananas so they got a small slice and she wasn’t having it. I thought she was being picky and gave her a slice of cucumber and went to bed. When I get up she’s sitting at the front of the cage puffed up and miserable, no begging or eating as usual. I take her out and she cries (never cried this much) and starts trembling.
Long story short at the vet they are stumped, her teeth are amazing for 5yrs old (thank you very much) and X-ray shows some gas but physically she’s not bloated and abdomen is soft. Temp is low but still normal. We get a very clear urine sample and specific gravity is 1.03 😔 she cries when you palpate her bladder but no blood or infection and X-ray showed no stones or sludge. Her lungs and heart looked and sounded good. Small ulcer on her right eye(from kidney failure?) that I am putting erythromycin in.
Here’s the plan gabapentin for pain and simethicone and critical care as well as cisapride. Also subq fluids.
My problem is she is just sitting there all night eyes wide open, is she in that much pain or is smthn else also going on? I’ve never had a pig with eyes like this it’s real weird, she won’t close them or relax, did she have a stroke or smthn? What else can I do to keep her comfortable?

Hi! I am ever so sorry!

Your girl is in major pain which is located somewhere in the body. :(

If there is nothing else you can do seeing that she is already on pain relief and support feeding and that your vets have reached the end of their line, you may need to consider whether it would the be kindest gift to release her from her suffering. Sometimes it is sadly the most loving, but also the most heart-beaking gift we can make when vet care and home support are not enough.
I know myself only too well how hard it is to make that decision; having been there often enough with piggies of my own, including a same aged sow whose kidney was going seriously wrong. :(

For any medical advice I am linking in some more experienced members: @helen105281 @Jaycey @Abi_nurse @Freela

This is a bit beyond my expertise. I agree with Wiebke that it my be kindest to let her go but I know you will want to have tried everything. Is her pain relief twice a day? did the vet say if there is any scope to increase the dose? I am not familiar with dosong on Gabapentin but I know Abi who has been tagged is. I'm sorry she is so poorly.

@Cookies&Glitter How is her poop output? Piggies can go into gut statis and not be bloated. It can be very painful and puts them off eating.

One of my boys is currently going through it. He wasn't wanting to eat anything and just couldn't get comfortable. He's been on gut stimulants and pain relief for a few days and he now seems brighter. He's begging for his food now although not eating as much.

I’m hoping that’s a big part of it. She up and moving and eating a little today, nothing near her normal begging but nibbles of shaved cucumber skin and carrots. Still pretty subdued but I’m hoping she at least perks up, I don’t want her to be miserable.

Please make sure that you feed enough and often enough! You can find information on gut stasis, and how to spot it in this guide here: Bloat, Gi Stasis ( No Gut Movement) And Not Eating

It is very difficult in that band when you are not quite sure whether to euthanise or not yet, when any decision is full in your own judgement and you do not know exactly what is wrong. There is no right or wrong, as long as any decision is made with your piggy's wellbeing topmost in mind. :(
If in doubt, follow your gut.

Night and day difference. Couldn’t even get all the subq fluids in today and she’s up and eating and was trying to run away from me. I am so happy and trying to find a new specialist to take her to, I know she’s getting old but I am just happy she’s not in pain.
