Blood Spots. At a loss

My Bercy (3.5) has been having little blood spots turn up in his urine (?) for about two months. No wounds or anything on his feet. He saw a vet who prescribed antibiotics, to no avail. The tricky part is that when we did X Rays to check for any sort of stones, they came back completely normal. We did another round of antibiotics as his vet believed it was a “prolonged bladder infection.” Thankfully this temporarily worked, and they disappeared for about a month. The past few days they’ve returned. They’re everywhere. It’s odd because they’re just tiny little spots as opposed to puddles or splotches. He now seems to be losing a bit of fur around one eye as well. I’m so worried. He sees the vet again tomorrow morning, but I’m just wondering if anyone has had this experience, and what it turned out to be? I have no idea what to bring up tomorrow or suggest we do, god forbid the vet is as confused as I am. They also haven’t eaten any fruit/veg that’d cause this pigment. Thank you

Does he display any pain when peeing?
One possibility ti bring up with your vet is a bladder condition called sterile cystitis (it’s similar to the condition seen in cats). This is not a bacterial condition so while Antibiotics can sometimes suppress the symptoms, they won’t cure it as there is no bacteria to cure. Symptoms will continue to return in waves. It is a condition which is only diagnosed by ruling everything else (ie bacterial infection and stones) out first and then seeing if symptoms continue.
There is no cure for it (although a mild case may self resolve but it can take years) and it is a long term condition but management is with daily painkillers and glucosamine.
